You can use our Online Service - Emis Patient Access to:
- Book appointments
- Request repeat prescriptions
- View part of your medical record (allergies and immunisations)
To register for this service please come to the surgery to collect a form to complete and register, when you hand it back in we will need a form of photo ID.
Patient's that were registered with our previous Online Service will receive a letter in the post informing them of the new login details.
Please be aware that you can only use Emis Patient Access for these facilities. Please do not try to use other apps for these types of service.

We can now send your prescription requests electronically to the pharmacy of your choice. This is by far a more secure system with no need to collect from the surgery, you simply collect direct from the pharmacist. If you wish to sign up for this, please complete a 'Nominations Form' in Reception or click here to download and return to our Reception Team. Once we are fully up and running on the system, in the coming weeks, we aim to set up patient 'on-line' ordering of medication, via the website. Watch this space!
Text Messaging Service
We are now able to text information to you rather then sending you a letter. This can be used for routine health questionnaires, notifications on your annual health checks, smoking status and much more. Messages can also be sent to 'landlines', with the patient needing to enter their date of birth on the phone to obtain the message to ensure that we are contacting the correct member of the household. Please ensure we have your up to date phone numbers.

We accept all new patients and through the natural process of new babies welcome family members living with existing patients, as The Chestnuts Surgery prefers to maintain the health care throughout a household.
With this in mind, if you wish to join the practice please submit your request in writing, stating your full name and date of birth, along with the name of the existing patient you live with. We will send you the necessary form to complete and invite you to attend the surgery for a new patient medical.
Please note: you are not considered to be registered with the surgery until you have had a new patient medical - if you do not attend after 30 days we will inform the Health Authority whereupon your details will be returned to them and you will remain registered at your previous practice.
The Chestnuts is one of the few remaining Huguenot houses in Sittingbourne and there has been a medical practice on the premises since 1811. The house was extended in 1981 to accommodate five new consulting rooms and modernisation of the existing facilities.
Did you know that you can now register for online services, which allows you to request prescriptions and book appointments online. All you need to do is pop in to the Surgery with photo I.D. and fill in the form.
